Once you have booked your first appointment please complete the new client form below to let us know the reasons for your visit, any relevant medical history or any details of what you would like to work on. If you are not able to fill in the form online, you are welcome to do so at the clinic (please allow some extra time  to do this before your treatment).  New client forms are available at our reception.

It is important for us to be able to tailor your treatment towards your specific needs, but also to be aware of any contraindications to your massage.
